Medical professionals refer their patients to Wellness Resolutions. Wellness Resolutions is a well established team of Registered Dietitians and Licensed Dietitians and Nutritionists.  We receive referrals from a variety of medical professionals including but not limited to:

• Primary care physicians
• Nurse practitioners
• Physician assistants
• Gastroenterologists
• Endocrinologists
• Cardiologists
• Nephrologists
• Orthopedists

We can assist your patients with:

• Weight loss
• GLP-1 Management and precertification visits
• Bariatric surgery screenings and post-op care
• Diabetes management
• Lipid management
• Blood pressure management
• Digestive disorders including IBS, SIBO, GERD, celiac and gluten sensitivities

We accept all major insurance plans in the state of Rhode Island. Our services are often covered at 100% with no out of pocket expense to the patient. Our office will verify insurance eligibility, copays and deductibles for each patient prior to their first visit.

We offer both in office and virtual telehealth visits via secure Zoom conferencing.  As for telehealth, we are licensed to treat patients via telehealth in Massachusetts, Rhode Island, Connecticut and Florida.

Referral Process

To refer a patient to our practice, simply fax your standard referral form to 401-305-6617. Once we receive the referral, we will contact the patient to schedule their appointment.

Please be sure your referral includes the following information:

• Patient’s Name, Address, DOB and Phone Number
• Patient’s Insurance Provider and ID #
• Patient Diagnosis
• Referred by (name, phone & date)
• NPI of referring provider
